Stentless Bioprosthetic Aortic Valve Insufficiency in an 80-Year Old LVAD Patient

An 80-year-old man with a history of bicuspid aortic valve complicated by severe aortic insufficiency presented with progressive NYHA class III symptoms and severe bioprosthetic aortic valve insufficiency. Transcatheter aortic valve replacement was planned via transfemoral approach. The authors encountered several technical challenges, and describe them herein.
